Take these steps to resolve:
Verify at your Mitel PBX that you have enabled HCI/TAPI Values for monitored and Call Control at Phones Class of service.
Proceed to install the vcredist_x86.exe. The file can be found at: http://www.microsoft.com/Downloads/details.aspx?FamilyID=32bc1bee-a3f9-4c13-9c99-220b62a191ee&displaylanf=en
Download and install file.
Stop and start the Quality Management Services and recheck the Call Control Log to ensure that Mitai loads correctly.
