As part of the evaluation process you can add notes to the evaluation.
General Notes - Add notes that may apply to the whole evaluation by using the General Notes field
Question Notes - Add notes to each question by hovering over the text of the question and clicking on the 
button that appears to the left of the question. You can remove a question note if you hover over the note and click on the 
button.
Bookmark Notes - Add notes to each bookmark by clicking on the Bookmark Notes field that appears below the bookmark
